Health-tech recruiters describe the typical band for a full-time named CSO as £70,000–£100,000, with current mandates advertised at £75,000–£95,000.2 Consultancies quoting for in-house senior CSO capability put the fully loaded figure (salary plus National Insurance, pension and overheads) at £90,000–£140,000.3 The working range for a private-sector standalone CSO is therefore £75–90k, plus roughly 25–30% on-costs on any employed hire.
The day-rate market: £900–£1,100 for a named CSO
Buy CSO time by the day and the numbers climb quickly. Specialist clinical safety consultancies publish rates of £900–£1,100 per day where the consultant acts as the named, accountable CSO on your file, with leading UK practices quoting £895–£1,050 and senior strategic input from £1,000/day upward.4 Short interim contracts occasionally advertise lower, but those rates buy availability, not accountability.
The arithmetic is unforgiving: two consultancy days a week cost roughly £95,000 a year: standalone-hire money, with nothing retained in-house when the engagement ends.
The embedded route: the dual-role clinician-CSO
There is a third model, now standard practice in specialist providers: an existing senior clinician takes on CSO accountability alongside their clinical role. The standards actively anticipate this: the CSO must be a practising clinician, and nobody knows a service's pathways, patients and failure modes better than the clinician already running them.
The going rate for that uplift is £2,500–£3,500 per month (£30–42k a year), at the lower end for GP-scale services, rising for larger organisations, regulated portfolios and AI deployments. It is how much of the independent sector now meets the mandate, for reasons that are structural rather than merely financial:
- Full accountability, no vacancy risk: no recruitment cycle, no 15–20% recruitment fee (£12–18k on an £80k post), and no notice period before cover exists.
- Continuity: the accountable clinician is in post permanently, not shared across a client book or present two days a month.
- Clinical context included: the dual-role CSO wrote the pathways the software is being deployed into, context no external signatory can rent.
- Inspection-ready: CQC and commissioners see a named internal clinician, not an outsourced signature.
